Registering with the Practice
As long as you are living within the boundaries of the practice's catchment area and provide us with photographic ID and proof of your home address you can register with us. We do not discriminate in any way against patients wishing to register with us.
To register, come into one of our surgeries with ID and proof of address and complete a registration form. On completion you will be asked to make an appointment with either a nurse or a doctor for a new patient check.
Download registration forms
You can download the forms you need to register from here. This download includes the NHS registration form and our new patient questionnaire. You will need to fill in both forms for each patient who wants to register.
These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.
Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.
